Our Clinical Assessor and experienced Nurse team deliver safe, person‑centred clinical support. We work with families and commissioners to assess needs, create care plans and provide the right level of nursing input.
Services include:
- Clinical assessments and care planning (risk assessments, reviews, outcome goals)
- Medication administration and oversight in line with the care plan and MAR chart
- Clinical observations and monitoring (vital signs; escalation pathways)
- Wound and pressure‑area care, dressings and skin integrity checks
- Continence care including catheter support (urethral/suprapubic) and stoma care
- Enteral/PEG feeding and nutrition support where clinically indicated
- Seizure support and rescue medication as prescribed
- Training and competency sign‑off for support workers/family (meds prompts, infection prevention, moving & handling)
- Liaison with health professionals (GPs, community nursing, therapy teams) and clear documentation
All interventions are delivered following individual assessment and competency sign‑off.
We provide planned and responsive visits; we do not replace emergency services.
