Our Physiotherapy and Home Assessment Service brings qualified clinical expertise directly into the environments where people actually live addressing the greatest barriers to recovery, independence, and safety where they exist, and building the confidence and capability to overcome them.
Led by our Clinical Physiotherapist and Home Assessment Lead, this service provides comprehensive assessment, structured rehabilitation, and practical, evidence-based advice all tailored to the individual. Whether someone is recovering from surgery, managing a long-term neurological condition, at risk of falls, or simply struggling with daily movement at home, our approach is the same: understand the whole person, understand their environment, and build a plan that works for both.
This is a responsive, coordinated service that fits around existing care arrangements, communicates clearly with every professional involved, and keeps the individual and their family informed and in control at every stage.
Who Is This Service For?
Our Physiotherapy and Home Assessment Service is suitable for a wide range of children and adults, including those who are:
Recovering from surgery or hospital admission including hip and knee replacements, fracture repairs, cardiac or respiratory procedures, or any period of deconditioning following a hospital stay.
Living with neurological conditions such as stroke, Parkinson’s disease, multiple sclerosis, or acquired brain injury, where movement, balance, and functional ability may be affected.
At risk of falls, or who have experienced a fall including those with a history of near-misses, reduced confidence following a fall, or identified risk factors such as balance impairment, muscle weakness, or unsafe home environments.
Managing long-term mobility or musculoskeletal conditions including arthritis, chronic pain, or progressive conditions affecting strength, posture, and movement.
Receiving care at home or in supported living where moving and handling practices need to be assessed, optimised, and embedded safely into everyday routines by carers and care staff.
Older adults wishing to maintain independence who want to stay well, stay active, and remain safely at home for longer.
Families and carers supporting someone at home who need guidance, practical training, and confidence to support their loved one’s mobility safely between professional visits.
This service is suitable for individuals known to statutory services and those arranging support independently.
Eligibility Criteria
Referrals are accepted from Local Authorities, NHS Continuing Healthcare (CHC) teams, families self-funding or holding a Personal Health Budget (PHB), and via discharge planning from acute hospital or specialist inpatient settings. Eligibility is determined through a comprehensive needs assessment conducted by our senior clinical team, using the Care Act 2014 framework for adults and the Children and Families Act 2014 for young people.
We accept placements at short notice where safeguarding risk or carer breakdown requires an urgent response.
What Does Physiotherapy & Home Assessment Support Actually Look Like?
Every package of support is individually designed. There is a structured, clinical process that ensures the right elements of support are in place for each person. Here is what that can include:
Functional Movement and Falls Risk Assessments A thorough clinical assessment of how the individual moves, transfers, and manages daily tasks at home. Where a falls risk is identified, a clear, written action plan is produced with specific, prioritised recommendations for the individual, their family, and any care team.
Rehabilitation Programmes Structured, goal-led rehabilitation for individuals recovering from surgery, managing neurological conditions, or rebuilding strength and function following illness or deconditioning. Programmes are designed to realistic timescales with clear milestones and regular progress reviews.
Personalised Exercise Plans Individually tailored exercise programmes designed to improve strength, balance, flexibility, and confidence. Plans are presented in accessible formats and include guidance for carers and family members to safely support practice between sessions.
Moving and Handling Assessments and Safer-Transfer Training Formal assessment of how an individual is currently being moved, repositioned, or transferred identifying any unsafe practices and recommending improvements. Practical training is provided to carers and care staff to embed safer techniques and reduce risk of injury to both the individual and those supporting them.
Equipment and Adaptations Recommendations Clinical recommendations for mobility aids, seating, grab rails, hoists, and other adaptive equipment matched to the individual’s specific needs and home environment. We liaise directly with occupational therapists and equipment suppliers to ensure recommendations are acted upon efficiently and without unnecessary delay.
Home Environment Assessments A practical review of the home environment including access routes, layout, flooring, lighting, and daily routines to identify hazards, suggest modifications, and support the individual to live as safely and independently as possible.
Carer and Team Training Structured training for family carers and care staff to ensure that exercise routines and safe practice are properly embedded between professional sessions. When everyone involved understands the plan and their role in it, outcomes are significantly better.
Reports, Reviews, and Professional Communication Clear, well-structured written reports are provided following assessments, with copies shared with the referrer, commissioner, and any other relevant professionals. Regular reviews ensure the plan remains appropriate as the individual’s needs change.
Multi-Disciplinary Coordination We work alongside not in isolation from existing health and social care teams. Our Clinical Lead liaises with GPs, consultants, district nurses, occupational therapists, social workers, and care managers to ensure everyone is working to the same goals and that the individual receives a joined-up, coherent package of support.
