What Is Homecare Support?
Home is more than a building. It is routine, comfort, familiarity, and independence. For many people living with a disability, health condition, or age-related need, the ability to remain at home rather than moving into a residential setting is a priority that shapes everything else. Our Homecare Support Service exists to make that possible.
We provide skilled, flexible care and support to children and adults in their own homes, delivered by trained professionals. Support can begin at as little as two hours per day and scale to regular visits throughout the day or continuous, round-the-clock care, whatever is required to keep the individual safe, comfortable, and as independent as possible.
As needs change, whether gradually over time or suddenly following a hospital discharge, health episode, or change in family circumstances, your care package can be reviewed, adjusted, and reshaped to reflect your new reality. We design the service around the person.
Our experienced risk-assessment team will visit you at home to carry out a thorough assessment. It is a genuine conversation about who you are, what matters to you, what you can do independently, where you need support, and what your goals look like. Everything that follows is built on that foundation.
Who Is This Service For?
Our Homecare Support Service is available to a wide range of individuals across all ages and circumstances, including:
Children and young people with complex health or care needs whose families require skilled professional support at home to assist with specialist care tasks alongside their own caring responsibilities.
Adults with mental health conditions who need structured, compassionate support in the home environment to maintain stability and day-to-day functioning.
Adults with learning disabilities who benefit from consistent, skilled support at home to navigate daily tasks, maintain wellbeing, and live as autonomously as possible.
Adults with physical disabilities who require practical support to maintain independence, manage their daily routines, and engage with the life they want to lead.
Older adults who need assistance with personal care, medication, meals, or daily tasks in order to remain safely and comfortably at home whether living alone or with family.
Individuals following hospital discharge who need short-term or longer-term care at home as part of their recovery and rehabilitation.
Whatever the diagnosis, background, or circumstance, the question we start with is always the same: what does this individual need to live well at home?
Eligibility Criteria
Referrals are accepted from Local Authorities, NHS Continuing Healthcare (CHC) teams, families self-funding or holding a Personal Health Budget (PHB), and via discharge planning from acute hospital or specialist inpatient settings. Eligibility is determined through a comprehensive needs assessment conducted by our senior clinical team, using the Care Act 2014 framework for adults and the Children and Families Act 2014 for young people.
We accept placements at short notice where safeguarding risk or carer breakdown requires an urgent response.
What Does Homecare Support Actually Look Like?
Every homecare package is unique, but all share the same commitment to being genuinely responsive to the individual.
The Range of Support:
Our Homecare Support Service covers a broad spectrum of care and practical assistance, including:
- Personal care Support with washing, bathing, dressing, grooming, continence care, and all aspects of personal hygiene, delivered with dignity and respect at all times.
- Medication administration Safe, accurate support with prescribed medication, including the administration of controlled drugs where a care worker is trained and authorised to do so, in line with individual medication management plans.
- Assistance with mealtimes Preparation and assistance with meals and drinks, taking into account dietary requirements, cultural preferences, swallowing difficulties, or specialist nutritional needs.
- Help with daily tasks Practical support with the everyday tasks that underpin independent living: managing correspondence and bills, accessing services, supporting attendance at appointments, and maintaining a safe and comfortable home environment.
- Specialist care For individuals with complex or clinical care needs, our team can provide a higher level of specialist support, including post-operative care, end-of-life care, complex health condition management, and support for individuals with challenging behaviour. This is discussed and assessed on an individual basis.
Flexible Hours and Scheduling:
- Care packages start from a minimum of two hours per day, making it possible to provide targeted support even where needs are relatively light.
- Visits can be arranged at the times that matter most to the individual morning routines, mealtimes, evening preparation for bed, or overnight support.
- For individuals who require more continuous oversight, live-in care or 24-hour rotating support can be arranged as part of a bespoke package.
- Packages can be structured on a short-term basis (e.g., post-discharge step-down care) or as a long-term ongoing arrangement and reviewed regularly as needs evolve.
Your Care and Support Plan:
Prior to the commencement of any support, our risk-assessment team will visit the individual at home to develop a detailed, person-centred care and support plan. This plan will document:
- The individual’s assessed needs and agreed outcomes
- Daily routines, preferences, and priorities
- Communication needs and preferred methods
- Health conditions, medical history, and clinical care requirements
- Medication and health monitoring protocols
- Risk assessments, including moving and handling, falls, skin integrity, and environmental safety
- Emergency procedures and escalation pathways
- Family and carer involvement and wishes
This document is reviewed regularly, updated when circumstances change, and shared with all relevant parties always with the individual’s consent.
