Our Clinical Physiotherapist & Home Assessment Lead provides assessment, rehabilitation and practical advice to help people move well and live well at home.
How we help:
- Functional movement and falls risk assessments with clear action plans
- Rehabilitation programmes (e.g., post‑operative, neurological and mobility support)
- Personalised exercise plans with goal setting and progress reviews
- Moving & handling assessments and safer‑transfer techniques
- Equipment and adaptations recommendations (e.g., mobility aids, seating, rails, hoists) and liaison with OTs/suppliers
- Home environment assessments to improve access, layout and daily routines
- Carer and team training to embed exercises and safe practice between sessions
- Reports and reviews for families, commissioners and referrers
Support is tailored to the individual and coordinated with existing health and social care teams.
Brief & match; vetting and compliance (DBS, right to work, references, professional registration where relevant); introductions and scheduling; quality oversight and clear reporting. We coordinate with existing teams so everyone works to the same plan and outcomes.
